This chart allows students to track the moon's phases for 14 days. I used this as a project in my science class- students tracked the moon at home each night and turned it in after the 14 days. It could be used as extra credit or even whole group (there are websites that will tell you the moons current phase)!
In each circle, students draw what phase they see and then label the phase on the line below the circle.
The chart starts on a Monday and ends on a Saturday
There is also a HOTs question for students to answer below the chart.
