I like to send home both these surveys at the beginning of the school year. The computer survey lets me know which students will have access to internet, and opens up the possibility of using email as a line of communication for parents. The second "interest" survey often yields very surprising, candid results! Since it doesn't look as "official" as a school form, parents often open up and share everything from their child's learning difficulties to information about what's going on at home that may affect the student's behavior to severe allergies that they didn't not mention on the nurse's form!
I'm including these surveys as a Word document, so that you may edit the questions as you deem necessary. For example, questions #1 and #2 on the interest survey ask for the birthplace of the child, and how long he/she has lived in NY.
