This information form is the one I use on the first day of school each year. It is generic enough to use in all grade levels, though there is a section for student's to fill out their elective schedules. This form asks for important details such as parent contact information, transportation, and allergies. The second page (I print one page front and back for each student) contains a "get to know you" section that asks about favorite/least favorite classes, what extracurricular activities the student is involved in (so I can plan to attend and support them), and what motivates the student. There are also two questions that ask students to tell me something that I should know that will help me be a better teacher for them during the year, as well as a place for students to ask me questions. I usually collect the papers on the first day and answer a few questions they ask each day during the first week of school. They enjoy learning about me, just as I am learning about them!
This is a great way to have all important information in one organized place. I keep the all information forms in a file on my desk so they are easily accessible throughout the school year.
