Product Description
This is a behavior chart that I created for a fourth grade student with an emotional disturbance. The 5 target behaviors are sitting appropriately, speaking appropriately, staying on task, keeping hands and feet to self, and following directions. Every morning I met with the student and we decided on a goal as to how many check marks he needed to earn in order to earn a reward. Then he selected from a menu of rewards (that we decided on in the beginning of the year) and we wrote down the reward he was working to earn that day.

If this is your student's first behavior chart do not use five target behaviors- start with 1-3 depending on age and severity of behavior.

Be sure to put your student's name where I have marked "student".
