This 5 point scale evaluation sheet allows you to look at the results from the 5 point scale trigger checklist and really work with some of the target areas to make improvements and strategies. This can allow a young person to feel in control of their emotions and help aid them in the skills needed to feel calm, in control and regulate those emotions.
