These 5th Grade Math Assessments are just what you need to keep track of how your students are doing with each of the Common Core math standards. Do you like to give a weekly math quiz? Do you need a quick way to gather data to help you adjust your small groups? Do you need data on each standard for your standards-based report cards?

Then this Math Assessments pack is for you! To really assess understanding, assessments should offer a variety of question formats, and these formatives do just that. The questions match the rigor of the Common Core standards and allow students to apply their skills and understanding. They are NOT the same question repeated 8 times with different numbers.

NOTE: If you already own this resource, please re-download. It has been completely updated with two versions of each assessment and new Teacher and Student record-keeping tools.

  • 2 Versions of Each Assessment There are 40 unique assessments, at least one (sometimes more) for EACH of the 5th grade Common Core math standards. That's enough for one a week through the school year. AND, there's two versions of each assessment--a FORM A and a FORM B. They test the same skills, but use different numbers. Use the two forms as pre/post assessments, or as a retest after you've done some reteaching.

  • Teacher Data-Tracking I've included two versions of a grade sheet to record your students' scores. One version is an editable PowerPoint file - just type in names and data. The other .pdf version allows you to print and write in the data.

  • Student Data-Tracking Again, two versions - one with color, one without. Use these in your students' data notebooks or separately. See the preview for a closeup look at all the data tracking forms.

  • Detailed TOC and Correlations Table - helps you quickly find the formative you need for the standard(s) you're teaching.

  • Answer Keys - for both Form A and Form B assessments.

  • Binder Cover and Spine

Classify two-dimensional figures in a hierarchy based on properties.
Understand that attributes belonging to a category of two-dimensional figures also belong to all subcategories of that category. For example, all rectangles have four right angles and squares are rectangles, so all squares have four right angles.
Represent real world and mathematical problems by graphing points in the first quadrant of the coordinate plane, and interpret coordinate values of points in the context of the situation.
Use a pair of perpendicular number lines, called axes, to define a coordinate system, with the intersection of the lines (the origin) arranged to coincide with the 0 on each line and a given point in the plane located by using an ordered pair of numbers, called its coordinates. Understand that the first number indicates how far to travel from the origin in the direction of one axis, and the second number indicates how far to travel in the direction of the second axis, with the convention that the names of the two axes and the coordinates correspond (e.g., 𝘹-axis and 𝘹-coordinate, 𝘺-axis and 𝘺-coordinate).
Solve real world problems involving division of unit fractions by non-zero whole numbers and division of whole numbers by unit fractions, e.g., by using visual fraction models and equations to represent the problem. For example, how much chocolate will each person get if 3 people share 1/2 lb of chocolate equally? How many 1/3-cup servings are in 2 cups of raisins?
