I have a series of skill worksheets and matching assessments that I have created for my reading instruction. In general, I focus on one reading skill each week (this particular skill is on Cause and Effect) - after three days of mini-lessons, I give the kids a Practice Worksheet on Day 4 (see my store to purchase Practice Worksheets). They will be taking a graded Skill Assessment on Day 5 of each week. This particular assessment focuses on using identifying cause and effect relationships in text. While most of the questions on this assessment focus on that particular reading skill, I always have sections for general comprehension questions and context clue reviews. Look for my other skill Practice Worksheets and matching Skill Assessments. It makes assessing reading so much easier and as close to the state tests as I've personally seen.
