This tracking document is versatile and can serve many purposes. One example of implementation is for teachers to input the dates that each standard is taught and/or reviewed, which is ideal for planning, pacing, documenting small group instruction, modified instruction, meeting the guidelines for student IEP/ 504 plans, and demonstrates the use of data for evaluation purposes when utilized to meet the needs of your students. Color coding proficiency levels, entering proficiency levels, and/ or entering scores can all be inputed to track your students. When I assess my students I color code the students' proficiency level with highlighters and then write the date of the assessment over the highlighter in each score box. This allows me to see progress over time and the dates that I reassessed my students.
