I like to have these student information sheets available on the first day that I meet my students and parents during our School Orientation. This paper is extremely important for me throughout the year as I use it as a reference for how my students go home and I use it for contacting parents. I have decided to post different mascots, because I like to have my own mascot on the sheet. I have a shark, a dolphin, a raccoon, a tiger, an alligator (or crocodile), a rattlesnake, a lion, a bee, a leopard, and a bulldog. When I go on field trips, I like to take my class set of these and shrink them down to 25% of the image on my copy machine at school. I trim the pack of shrunken papers on the paper cutter and staple the small pages together. This gives me a tiny version of all of my parent contact information in case there is an emergency.
Please view the mini windows where you can see the ENTIRE download. The only addition are the other black and white versions of the animal mascots which are miniature at the top left hand corner of the page.
