Use this worksheet at the end of each week to have students evaluate their learning and inform parents of their students performance in school. While completing this sheet, students have to think of the best thing that happened in school that week, something they learned, something they look forward to next week and consider if there is something they think they could do to be better in school. Once students evaluate their learning, the teacher evaluates the students performances, behavior, and homework completion.
I've been using this form for a while and think it is a great learning tool for students since it forces them to recall what they did for the week. It also allows me to inform parents of their child's performance on a regular basis. It takes me less than 5 minutes for me to complete my part of this sheet for the entire class.
