An ABC Chart is a direct observation tool that can be used to collect information about the events that are occurring within a student's environment. "A" refers to the antecedent, or the event or activity that immediately precedes a problem behavior. The "B" refers to observed behavior, and "C" refers to the consequence, or the event that immediately follows a response.
This product was designed to work with Google forms to collect all of the data you need to try and pinpoint specific triggers for behavior. The form includes basic information about the child, plus information about each incident, to include: date, time, duration, possible antecedents, measurable behaviors, severity, consequences, the child's response, and the location of the event. There is also room for notes.
This form can be customized and edited and data are stored in a Google Sheet for sorting, graphing, and comparing information.
