I use this tool almost daily. I have some high-hitting behaviors in my classroom and this straight forward, simple "ABC" chart helps me track and examine what is motivating my student's behavior and create a more appropriate Behavior Intervention Plan that will change my student's behavior. It also helps me see when I need to adjust my plans when new behaviors arise.
At the top of the chart I have the behavior terms that all special education teachers (of self-contained classrooms) are familiar with. I included a small "definition" underneath to help those perhaps not as familiar with the terms. (Handy to have your IAs/para-educators fill out the form when they deal first hand with the behavior.)
For my classroom I have a binder dedicated solely to behavior, each student has a tab and at the beginning of the year I fill the tab with a few blank forms (or several depending on the kiddo :) ). This system is quick and efficient, and it's easy to look back and see all "big" behaviors in one place.
This could also be a helpful tool for general education teachers who have some more significant behaviors in their classroom.
