Fitness Game was created for high school classes, but could be used in middle school as well. I have used this in PE classes with 18 - 60 high school students and was successful in all.
Review the exercises before using, I would suggest teaching the different exercises prior to this activity to make it go smoother. Another option would be to change/adjust the exercises to better suit your classes.

-ACTIVE Bingo Rules and Team Rewards I used. (Rules are listed below to see before purchasing)
-10 completed ACTIVE Bingo boards with the exercises listed on them. Each of the 10 bingo boards is different. The boards are 6x6 so there are 36 exercises total on each board. There is a list of exercises I used for the students to draw from during the activity and not all of the exercises are on the boards so some will not be used.

ACTIVE Bingo Rules
•4-6 people per team (or whatever works best for your class).
•1 person at a time runs to the other side of the gym to select one exercise from the envelope/folder at a time. Each group member will rotate being the runner.
•Once the runner brings back an exercise, the entire group must complete the exercise before marking off the box IF you have that exercise on your ACTIVE bingo sheet.
•If the exercise is not on the ACTIVE bingo sheet, your group does not complete the exercise and you send a different runner to grab another exercise from the envelope/folder.
•If you do not complete the exercise you could lose participation points for the day.
•Give 100% Participation
