Adapted student. Adaptations checklist. Adapted reporting. This is the report card insert for I use for students with adaptations to their educational program. It is designed as a checklist. Strategies used are listed under categories. The categories are distractible students, weak organizational skills, visual processing difficulties, testing adaptations, auditory processing difficulties, visual-motor processing difficulties, written language difficulties, weak reading skills, weak math skills, and allows a few lines for comments. At the beginning of each reporting period, I start one for each adapted student and keep it handy in my day planner binder. As I do adaptations throughout the term, I just flip to the student and check off what I've done. At the end of the term, it is done for report cards and it is accurate. The system is so efficient. The form is so practical and useful.
