I live by this slip, y'all! I use absence folders in my high school classroom to make students' absences easier on them AND myself. I used to think they were too time consuming, but once I started my system, it's actually time saving! The problem comes in when we have work that cannot be easily put in the folder. That's where these come in! Any time we take a test or quiz, do in-class reading, or turn something in -- anything that I can't physically put in the folder -- I fill out one of these. This way the students have no excuse for not knowing everything that they missed. The burden of responsibility shifts entirely to them. They know to bypass asking me and go right to the folder. :)
I've included a PDF with customized fonts as well as a word document so that you can edit it to fit your classroom needs.
