Inevitably you will have students who miss a day of school. It’s likely that you will need to send make-up work home with that student. At times, we even know about an absence in advance.
I created these forms to use in my classroom to for both unexpected and planned absences.
I’m always ready by keeping a stack of these forms on a clipboard, or in a folder on my desk. When a child is absent, I pull out the clipboard and jot down what we accomplish, as the day proceeds. Add the form to the items the child needs to complete the missed work, and my work is done.
If the absence is planned, having a blank form ready to fill in makes the planning and material gathering go more quickly as well.
Color and grayscale versions of each form are included.
