Do you need a way to help students personally own and take charge of their learning? This document is a self-reflection tool students can use to help improve their performance in your class.
This tool allows students, teachers, and parents to come together and identify a student's areas of strength in addition to areas of growth in hopes of improving academic performance. The form includes a list of about a dozen behaviors that a student should perform in order to be successful, and the student along with a parent should complete that list together indicating whether that specific behavior is a strength or area of improvement. I personally require students to fill out this form any time their grade falls below a C. Though I used it for math it could be used for any content area. I have included the Microsoft Word file so that you can edit as you see fit for your own specific class.
