Four sheets have been provided with this product. Choose the sheet that is most representative and useful for your situation and for that of your students and colleagues. In fact, different teachers may prefer different sheets for different students.
Each sheet could be printed on a different color to indicate which is which.
Additionally, sheets could be printed on different colors for different students. If your local printing company or education agency only offers a very limited number of paper colors, consider using highlighters and markers to help differentiate sheets for students.
Another option is to print these sheets on duplicate or triplicate paper (or simply use carbon paper) so copies can be retained for files or shared with stakeholders.
This sheet holds special value in the time period preceding and IEP meeting. This allows teachers to document the most effective and most used accommodation with a particular student. This sheet can help to narrow down the best accommodations for a particular student or introduce new accommodations.
In the final column, “Accommodation Efficacy & Student Success,” staff members can share their thoughts about how accommodations worked, difficulty/simplicity of implementation, the student’s response to the accommodation, the student’s grade on the assignment, whether or not accommodations were offered to any other peers, or suggestions for additional accommodations.
