This form is raved about by both teacher and parents. It is used to document accommodations and modifications used to complete graded work for students who are at-risk, students in the Response to Intervention process (RTI), students with disabilities or ESL students. The first page is an English/Spanish version. The second page is the main page with accommodations and or modifications used to complete assignments and a rubric to demonstrate how the grades was obtained. How often have you worked one-on-one to help a student complete an assignment but don't feel comfortable giving a grade of 100 based on the level of support you provided. The bottom of this page provides a rubric to use that helps you give a grade based on the level of support. The third page give another rubric with grades broken down even further. Parents love the level of communication about how their child is completing assignments and teachers love that they have a quick easy way to document accommodations and modifications.
