As a School Counselor, I use this form to document learning tools, accommodations and strategies that have been found to be effective for a particular student, even if they are not a candidate for a formal plan (IEP,PEP,etc.). The Learning Tools Form is simply a summary of tips and suggestions that have been recognized to help a student be their best learner. Classroom teachers will find the form very useful as well, as it is an efficient tool to document all the extra efforts made to help students thrive.
Documenting what works best for any student is a valuable resource for each educator teaching the student. This will serve as a supplement to communication you likely already have with your student’s former and future teachers. A written version should make for a more efficient process and facilitate a smooth transition for academic success in new classrooms or even a transition to a new school. You are welcome to share this with a student’s parent(s). A blank template, a completed sample and instructions for using the form are included.
