This is a survey/assessment I use with students on my caseload, and more readily the students I have in a Resource/Pull-out class where I see them readily and can work on building their capacity to understand their accommodations and advocate for them.
This bundle has the survey I use with the students, the rubric, and a spreadsheet template I use to track their progress.
I would recommend using this approach for students in secondary school (i.e. grades 6 to 12), and maybe more like 7th or 8th grade, but it all depends on the students.
