1. This screening tool is to be utilized as an aid in evaluating Body Management, Locomotor Skills, Manipulative/Object-Control Skills, Health and Fitness, and Social/Behavioral Skills of students in Kindergarten through 12th grade. Information gained from this inventory will be helpful in determining student Strengths and Needs.
2. These test items are to be compared to typically developing same-age peers. erbally Request student to demonstrate the following items. Provide a Visual Prompt/Example if the student is not able to correctly attempt any item. Also provide encouragement. Score a 0 if the student does not attempt or is unable to demonstrate the skill (i.e., 0 out of 2 attempts) score a 1 if the student is inconsistent in demonstrating the skill (i.e.,1 out of 2 attempts), score a 2 if the student is able to consistently (i.e., 2 out of 2 attempts). Add up the scores in each section. It is assumed that same-age peers would score the highest overall score shown with or without additional prompts or attempts. Include a note in the Strengths/Needs section of the report if the student required a visual prompt, multiple attempts, no attempt, etc.
3. Students who score 50-75% on any section should be further evaluated for Additional Supports that may be necessary in order to participate in physical education activities (i.e., accommodations, goals, verbal/visual/physical prompting, etc.).
