I have included files in the Zip Folder that include PowerPoint, Microsoft Word, and PDF. I included Microsoft Word so you can change the product to meet your needs.
I use the 4 minute 50 question fact test to assess students once a month. These tests are provided on my website. If a students passes the addition 50 question assessment (I allow two incorrect or 96%), but not the subtraction, I will only give them the subtraction 25 questions quizzes. Once students either pass the 50 question assessment in addition and subtraction OR make it through all addition and subtraction 25 questions quizzes, I move them onto multiplication.
I give the 25 questions quizzes 2 to 3 times a week with the same day or next day feedback. I pull small groups and talk about addition and subtraction strategies depending on what math fact they are currently on.
Students record their progress on the recording sheet provided and I use the teacher recording sheet to track student progress. When a student does not pass a math fact, they practice that fact, Students put a capital T in the box (for try again) if they do not pass. Once they pass the test, they put a capital P in the box in the row that corresponds to the math fact they currently are on or passed. Once a student reaches the XP (three tries), they must do extra practice. I use my I-Pad, flash cards, home practice sheets and other games to help students master their facts.
My students track their progress in their Leadership Data Notebooks. They set goals periodically throughout the school year in each academic subject. Once they have mastered their goal, they write a new goal.
