1. Against Education Advice (AEA) Form: Based loosely on the Against Medical Advice forms used in the health industry - this form can be used when a parent, family or student go against your education advice and decide to pursue a different path that could have consequences that may find their way back to you when/if they happen. While this form is not intended to be legally binding - it can help clarify what the position of your office or school is regarding issues that can become contentious or litigious.
2: Graduation At-Risk Notification Form: This form is also editable and can be used with seniors who are failing (or close to it) nearing the end of their senior year. Best when filled out with student and parent present at a school counseling meeting.
