This form is used to track the alignment of Reading instruction for all three tiers of instruction for each student. It allows all teachers involved with the instruction for each student to stay informed of all skills, strategies, etc. which are being provided in each tier of instruction. This form helps to ensure every student is receiving the correct amount of reading instruction necessary for their growth and progress. It helps to track the amount of time, the materials used in each tier, the person responsible for providing the instruction for each tier as well as the skills each tier will target. All interested parties can use this as a form of communication, as well as documentation and tracking of time each student remains in each tier of instruction.
