This assessment was built off of the Depth of Knowledge Wheel (Google this if you aren't sure what I'm talking about). The wording is very similar to Bloom's Taxonomy.
The assessment allows students to show what they know before beginning the unit, reflect on their knowledge, set goals, and develop a plan for reaching their goals.
My students are required to answer all questions even if they don't know the answers. I require them to answer all questions in complete sentences and make up what they don't know. This is a great "Ahh Hah!" moment for them when they realize later in the unit that they had no idea what they were talking about on their pre-assessment.
Then, they complete the post-assessment at the end of the unit. They also are given a reflection to discuss their growth and if they reached their goal.

The students love to take their Post-Assessment reflection/DOK responses and compare them to their Pre-Assessment responses to see how much they learned during the unit.
