This checklist has saved my life - no more randomly jotted notes, post-it notes, or other forms of trying to make anecdotal notes about Physical Education, Drama, Music, etc.
This EDITABLE PDF includes:
- Term, Subject, Date lines
- Learning Goal line (great for curriculum expectations)
- 3 lines labeled A, B, C (for specifics you are looking for within the expectation)
- 42 boxes that include student name section, 3 check-boxes that correspond with the expectations from above, and space to write down a small note
- a box at the end for overall expectations
I'm still all about paper documentation (I know, I know) - so I fill this out, print, and I'm ready to go. I can also fill it out, re-save it as a filled-in PDF, then use it on my iPad and make direct digital documentation on the file itself.
Example: I used this for Phys. Ed. Particularly Basketball.
I pulled out a curriculum expectation that went with what I was doing. Then, the three items I focused on was Dribbling, Passing, Shooting - at least that time. I've subbed in Participation, Cooperation, Teamwork, etc. on different occasions. Put in the students names, and off you go! While in the gym, you felt that little Johnny could pass and shoot the basketball, so you checkmark beside B and C, and make a small comment (great!). Then find he is not so great at dribbling. Make an X in the box (or a squiggle) and make a comment on it!
The EDITABLE PDF can have term, subject, date, learning goal, 3 looking-fors, and 42 names added easily with Adobe Acrobat.
NOTE: Just be sure to save each new checklist as it's own file before editing, or just keep this template and empty the file as you need it next time.
I also recommend printing off blank copies to keep on hand and put student names in as needed - especially handy for supply teachers!
