This score card is one that my sister, a licensed clinical social worker and I came up with :)
She and I put our heads together to create a tool she could used to keep track of her clients' progress. The categories in the first vertical column are descriptors for what she and I have deemed to be things that qualify anger. This is editable so feel free to modify it in any way you so choose.
This is a great way to document an individual's progress in an unbiased fashion. The terms used are concrete, observable behaviors and leaves little room for ambiguity.
You can use this in your practice or in the classroom.
Idea: Create an excel document with student/client name on a secure website. (Google docs with privacy set to specific individuals)
Paste this into the document. Create tabs with different dates. Now you have an electronic version that relevant parties can see without needing for you to dig for hard copies or sift through emails.
Includes an area or a signature.
Create a digital signature and insert to each file as needed.
