These Animal Report Forms come in two styles - one with handwritting lines and more simplified for younger students and ones with simple lines and additional information.
There is a Animal Report from for each of 6 animal classifications with short description of the animal class on each one - fish, insect, mammal, reptile, amphibian, birds, and a plain one.
Each animal report includes a spot for information such as what the animal eats, where it lives (both habitat and on a globe), what its young are called, how big it gets, interesting facts, a picture spot, and more.
With my own students I had them do a report on an animal from each of these classifications and then decorate their own cover to make a keepsake book of their animal discoveries. There are 3 covers includes (one in color and two black & white options for students to color).
