In preparation for our state test, I use the attached notes document with my students from the start of the year. This is editable to suit any quiz or test you are using in your classroom.
Students answer the question in their own words in the left column. In the right column, students support their responses with evidence from the text.
After we read a piece of text together, students complete the notes document independently (or with a partner). Then, we, as a whole group, go through each question. Students put their responses on the board. As a class, we discuss and add onto the notes.
Having used this notes system over the course of several years, I have found it helps students better comprehend each question's task along with how to answer each question.
I allow students to use this notes document on the assessment.
