In order to get the best staff possible, an advisor should consider using an application process.
With over 150 applications each year, it's very important to have a way to easily compare applicants. I've been a yearbook advisor for 20 years and have developed this form to make picking students for yearbook staff and much easier and more informed process.
This form includes basic information, questions about grades and disciplinary actions as well as agreements to be an active participant. Students are even required to write a short essay on the back of the form to give you a chance to view their writing ability and their ability to follow directions and complete the application correctly. A parent signature is also required and will come in handy should students not follow staff rules.
This file is in Word format for easy customizing to fit your needs.
Teacher recommendation forms are also included. (2 per page)
Forms without background and border graphics are also included.
