I love tracking progress of my students. I have found baselines for words and sentences, but none for reading that are quick and easy. That's why I created Reading Data Collection forms for different sounds in articulation therapy.

A lot of children with articulation errors might do fine in single words or sentences, but as soon as they get to reading and conversation, there is a break-down in intelligibility and in their articulation precision. But how do you show that they are making progress or need therapy when their baselines in words and sentences are 100%?

That's why I think these Reading Data Collection forms are a MUST HAVE in therapy. I use them to track progress, to write in progress reports I send home, to show the students that they are progressing, to use in my communication evaluation summaries, to help exit a child from speech or to qualify a child for speech services. These also tell me where a certain sound is hardest for them (initial, medial, final, blends, certain blends) and where I should focus my therapy.

The way I use them:
1. I print out 2 copies of the form.
2. I give 1 copy to the student and I hold on to 1 copy.
3. I have the student read their copy. As they are reading, on my copy, I put a mark through the words that they missed. Then I tally up their percentage for that certain target.
4. At the end of all the reading samples, I tally up their percentage overall on that sound.

Each target (initial, medial, final, certain blends) have 5-10 target words per reading exercise. This is so it is easy to calculate the percentage in your head.

The /S, Z/ Reading Data Collection form has 190 target words. This really gives you a good idea of where the child is at and can be done within 10-15 minutes.

I also use the same copy of the Reading Data Collection form to check the child's progress later. I just use a different color of pen to mark the words they miss as they read.
