Here are a couple of resources to use when collecting articulation data.

-Artic probes sheet - List all the phonemes you want to check in the left column, and take your data in the rest of the columns. This page is great for initial evaluations when you have a lot of sounds you want to check stimulability for.
-Artic data sheets - One option has 15 boxes per level, the other has 34 boxes per level (or 17 if you want to use the top box for the date and the bottom box for the %).
-Data graphs - A bigger option and a smaller option. These can be very motivational for students to visually see their progress.
-Ten box data sheet - Write the student's name or initials, the date, sound, context/level, etc. in the top box and take your data in the little boxes. This makes calculating percentages a breeze!
