This is a very simple data sheet I've used in therapy for students whose goals are to produce singletons and simple CV or VC shapes.
FAR LEFT COLUMN: "STRUCTURE"- I have put CV or VC here before. You can also put C or V, but I do it differently (see next description)
COLUMN- "CONSONANTS"- I list the consonants I'm working on down this column. For example I frequently target Ling 1 sounds at this level, so I would write /b,m,p,w/ down this column. I also put "isolation or 'iso'" if I want to target it as a singleton.
COLUMNS- "VOWELS"- I list the vowels I'm working on in the top row. I also put "isolation or 'iso'" if I want to target it as a singleton.
HOW TO TRACK 'CV' PRODUCTIONS WITH THIS CHART:
Find the consonant on the left hand side and find the square under the vowel that is being targeted. Place your tally-marks there for data! I total the productions in the "Notes" section below as well as other relevant observations. If you are tracking singleton productions, the easiest way I did it was the place it in the far right side, under an "isolation" column or row. The second page is better for keeping track of how a student produces a word.I put the IPA transcription in "production" and notes in "other". The word "level" at the top corresponds to word, phrase, or sentence level.
