This is the homework sheet I use for my middle school students. It is straight forward and it is not "childish" which my upper level students seem to appreciate. I fill out the top by writing in their name and the due date. Then I circle the number of the direction they should follow for their homework (say the word __ times, say each word in a sentence, etc.). During their speech session, I write down the words that they practiced and any "trouble" words that may pop up. I use the notes section to either write words of encouragement or strategy notes for parents.
I have tried many different homework worksheets and this is the one I keep going back to. It allows you to really tailor your students' homework to the sounds and words that they need the most practice with.
I print this worksheet on both sides to save paper.
