Two matrices that will make group assessment a breeze! I use these charts year round. It is a great tool to see where individual students are in comparison with the whole class.There is a place for lesson title, lesson description, and date at the top. There are spaces for student names and 4 standards to be assessed during the activity/ lesson. There is also a scoring key and comments section.
In my classroom, I type students' names in the matrix and print. I write in the assessment scores and notes.
FYI: Most matrices are used for predictable behaviors. Such as participation, listening, and skills that can be assessed visually. I have added an example of an activity that I have done with my class.
