The first sheet in this packet is for an individual student. This sheet works well when gathering data for an IEP or the RTI/MTSS process and problem-solving. You can also use this sheet to share information with parents at conferences or to summarize information for a student moving on to another teacher or school.
The second form in this packet is helpful to a classroom teacher trying to summarize his/her set of students testing information for a particular assessment.
The third and fourth pages in this packet are for recording the results of assessments that are given multiple times per year (Fountas & Pinnell, for example). One sheet is for an individual student and one sheet is for a group of particular students.
The concerning score is a column for teachers to write the score that indicates the point of “concern” on a particular assessment. This gives information to stakeholders who see the student’s score in the score column to give it a point of reference.
