Use this point sheet to help students monitor their impulsive and inattentive behavior. This particular point sheet has two check-in points. (Perhaps one before lunch/recess and one at the end of the day.) You may choose to have the teacher delegate points, or have the student self-report and then check in with the teacher to see if he or she agrees with the student's self-report. This intervention is meant to be positive, so initial goals should be set at or around 70-75%. As the student is successful at this level, goals can be increased in increments of 5%. In general, it s recommended that goals not exceed 85%. If the student is consistently meeting or exceeding their 85% goal, they are ready to "graduate" from the point sheet.
Please browse my other point sheets, as I have several that target different behaviors and have varying numbers of check-in points.
