This chart was created for use with a student needing to self-monitor or have a teacher monitor attention to ask. The point system is clear with the number of reminders needed to score in each section. This chart may be used in multiple ways:
1. Teacher/TA/Buddy student can record for the student and work towards a daily goal. This can be kept on file for behavior data or sent home for frequent parent communication.
2. Self monitoring with teacher--the student rates themselves throughout the day and the teacher does the same. The goal is for the student and teacher to agree at least 80% of the time to teach self-monitoring.
3. Independent Self monitoring--the student rates themselves throughout the day with a quick checkout conversation with the teacher at the end of the day.
