One of my assignments in my district is assisting the School Psychologist during play based evaluations to determine if students meet disability criteria as having an Autism Spectrum Disorder. To help me stay organized and ensure that I assess all the important areas, I created this checklist for children ages 0-6 to guide my play based evaluations and reports. I have used this checklist at least once a week for the past two years.
There are four areas that I specifically use to guide my evaluation:
-Non-Verbal/ Social Communication
Each area has a Yes/No check box to indicate if you saw the child do a skill. There is also an 'Example' box which I use to take numerical data and write specific examples. To help caregivers understand my report and evaluation better I like provide both specific examples and approximate percentages for each skill in my reports.
