What's the best way to learn about your students interests and their attitudes about reading? If you had all the time in the world, you could interview each one. Or you could use this DIGITAL or PAPER survey to learn to learn so much about them! Either way, use the information to help you connect your students with good-fit books or for data-collection.
The paper copy also includes a Data Record sheet for your use. I make a copy for each student (all 80+ of them!) and put it into a binder by class. Then, when a student and I meet to discuss what they're reading, I have a place to jot notes. It's also really helpful for discussions with parents!
