Everything you need to collect and track behavior data, all in one spot!

This document can be downloaded as NUMBERS (MAC) or EXCEL (PC). Each tab of the documents has a different data collection piece.

This document has many different options for behavior tracking depending on your needs. Each tab holds different data. In my own data collection, I use many for each child to have a variety of data points. In each tab, the tables are automatically color-coded by behavior goal or behavior type, and the total behavior and average behavior is formulated to calculate automatically.

NAVIGATING THROUGH THE DOCUMENT: You can delete tabs that are not needed, duplicate tabs if you need another one, and delete columns (and corresponding graphs) as needed. If you ADD a column, it will not automatically add a graph. You will need to program that yourself. When you run out of rows for the date, you can add more, but you may need to select the last cell of the programmed columns (for total and average behavior) and drag it down to continue the formula.

Tabs:
SMALL GROUP BEHAVIOR: This tab is for documentation of specific behaviors in small group settings. You input the date or week in the table followed by the number of occurrences for each specific type of behavior. The behavior collection tool on the last tab is a tool that can be used to collect this before inputting it. It is very time consuming, but this is what you want to use for students with severe behaviors or who might need alternative placements.

GEN ED BEHAVIOR: This tab is for documentation of specific behaviors in general education settings. You input the date or week in the table followed by the number of occurrences for each specific type of behavior. The behavior collection tool on the last tab is a tool that can be used to collect this before inputting it. It is very time consuming data to collect, but this is what you want to use for students with severe behaviors or who might need alternative placements.

BEHAVIOR CHART BEHAVIOR: If your student is on a behavior chart, this is the tab you will want to use. Each column is a different behavior goal, as specified on that student’s chart. The data you put in is the amount of time that day that they met that goal. So, for example, if they can earn 4 possible points throughout the day for following directions, and they only earned 3 points, then the data for that column would be put in as “75%” because that student earned 75% of the points for that specific goal.

TOOLS AND ACCOMMODATIONS: This tab is just a list of possible tools and accommodations that you may be using for that specific child. It is a good accompaniment to the more concrete data listed in the other tabs. I have left some tools and accommodations in the table for you to use as needed, but you can add/delete/manipulate as needed.

ABA BEHAVIOR DATA DRAMATIC BEHAVIOR: This is a simple behavior data collection tool you can use for when you need the most accurate depiction of a child’s behavior. One chart is used per day and a tally would be placed in that column with each occurrence that the child displays that behavior. This data can then be put into the SMALL GROUP BEHAVIOR or GEN ED BEHAVIOR tabs within this document.
