This freebie contains two versions of two different handouts to support teachers in obtaining information from parents at the beginning of the year regarding contact information, student information, and access to technology.

About This Product:
As many schools are moving towards utilizing quality technology systems to maintain student information and contact records, there sometimes still comes a time when you require a paper-copy of student information in your classroom. For the times you are in need of paper copy information regarding students, check out these forms to use or adapt for your classroom.
Two versions of each form exist because I created these forms before the expansion of my school's digital study information system. However, I still utilize the forms - using the second version of the forms.

In My Classroom:
About "Getting To Know Your Child":
I have used this form in the middle grades to obtain information from parents when my school’s digital program for information had issues with accuracy and detailed information. I would often find that parent phone numbers were inaccurate or were not the best contact number for the parent. By asking parents to complete this for, I not only obtained information regarding the student learning style, but also verified contact information. I ask for addresses because I like to send postcards throughout the year. I ask for email addresses and phone numbers to verify those the school has on file - often finding phone numbers have changed. I ask for parent signatures to use for comparing for verification against signatures on future papers. The form also asks for parent-generated information about the student's strengths, weaknesses, interests, etc. I usually pass out this form with an envelope with my information on it to have the parent return the form.
About "Getting To Know You: Technology":
I have used this form in the middle grades in a school with a BYOD (Bring Your Own Device) policy. I have created two version of the form. One that asks questions regarding student access to technology at home. And one that asks questions about students access to technology at home AND regarding devices to bring to school.
