Each packets is 3 pages.
The parent packet contains a contacts page and 2 pages of response questions that will enable you to learn more about your students from the parent perspective. I attach this packet to the Welcome Parents letter and ask that they return it as soon as possible. Examples of response questions include: "Is there anything you are particularly worried about at this time concerning your son/daughter such as a recent behavior, new habit, fear, or a particular difficulty, etc.?" and "Tell me something amazing about your son/daughter."
The student personal information packet contains basic information questions and more advanced questions including height, pants size, parent occupations, and prescription medication. Students fill this packet out in class to allow you to assess what personal information they know about themselves and what they need to learn. You can give this assessment monthly to check for information mastery.
