It's that time again! School is starting up soon and that brings in a whole new group of students and their families. I have created a two page document that I like to use at the beginning of the year to help me get to know my students while also learning valuable information about their parents or guardians. I like to use the term "valuable", because I keep these handouts all year long. I simply create hanging file folders for each of my students and slip these two documents in the file folder for future reference.

Parent/Guardian Information Sheet:
This is a great resource that allows you to know all of the important and confidential information such as phone numbers, address, e-mail, emergency contact, allergies and medications, etc. The document even asks about transportation to and from school.
Partner Interview Game:
I love this game and it's simple to facilitate and explain to your students. Every student needs their own copy of the worksheet to play the game. Pair your students into groups of two. Once they are in their group; they begin to interview one another. This means they write down information about their partner and vise versa. The best part is that they are making new friends in fun environment. Once the time is up, the partners stand up and introduce one another to the class by reading the script they fill out.
