Description

Use these back to school forms to start the year with strong parent communication and organization. These forms are ready to print, copy, complete, and hand out to parents.

Included:

Transportation List - Editable versions provided

The 1st Day Transportation form is best completed when the parents first bring their child to class or drop off supplies. The first day of school is extremely busy, so it's important to start your after-school transportation list early. This form includes spaces for how the student will go home on the first day, the rest of the first week, and the rest of the year.

Complete the Transportation List form at the end of the first week. This gives parents time to work out their plans and make any necessary changes. Be sure to keep a copy of this form in your sub folder!!

The specific pages for each type of transportation are helpful for lining the kids up during dismissal. It’s helpful to keep these in your sub folder as well.

Parent Volunteer Form - Not editable

This sign-up form communicates your desire to work together throughout the year. Parents can complete this form to let you know if, how, and when they want to volunteer. The form lists ways for parents to help in and out of the classroom.

Be sure to contact parents who express a desire to volunteer in person and inform them of any volunteer background checks and other forms that may be required by the district.

Student Information Form - Not editable

Parents are the best source of information about your students. Building strong parent-teacher relationships will help you meet the needs of all your students. Use this form to gather critical information about your students' strengths, weaknesses, medical concerns, and histories.

Missing Supply Letter - Editable checklist provided

Use the editable supply checklist as you and your students organize your materials in the beginning of the year to keep track of missing supplies.

The letter is an easy checklist form to communicate with parents about which supplies are still needed. Sending a form like this is easier, and often better received by parents because it lets them know that they are not the only ones who forgot about a supply or two. We all know how hectic back to school shopping can be, so it's important to make parents feel comfortable about a few accidental omissions.
