Back to School Survey | Fully Editable | Digital and Printable

Are you looking for a back to school, ready-to-go, digital or printable, get to know you survey? Then this resource is perfect for you. This detailed survey contains over 40 questions which will help you get to know your students. Both version are fully editable. The digital version is a fully editable Google Form which you can customize according to your classroom needs. The printable version is also fully editable in PowerPoint.

Make your classroom inclusive from Day 1. This survey includes questions about gender pronoun preferences as well as family dynamics that may include same-gender parents. 

Older students can complete this survey on their own. If you teach younger students, you may choose to involve parents to help students complete the survey at home.

If you are still teaching online, the Technology section in this survey will help you get to know your students’ preferences regarding the use of tech and access to tech available at home. 

Some of the topics covered in this survey include: 

• Basic Information (name, preferred name, gender pronouns, birthday)
• At Home (family information, quiet workspace, typical afternoon)
• Interests & Hobbies (skills, sports, musical instruments, books)
• Mindful Me (how I respond to my emotions (sadness, happiness, anger)
• My Learning Style (learning preferences)
• Technology (access to various technical devices, printer, preferences)
• French Class (strengths, struggles, goals)
• Additional Information (open-ended)

Both English and French versions are included in this product.

Both versions are fully editable.

Most questions in the digital version are set to “required” by default. 

You may choose to change that setting when you customize the form. 

Don’t want a question? Deleting is as simple as clicking on the question, hitting the little trash can icon, and done.
