I needed a quick way to assess my students on their basic skills! I wanted to see where they were when they entered my classroom and when they left my classroom! I created a simple binder to keep track of each student and their levels when it comes to basic math and phonics skills! (Grades K-3) There are TWO types of assessments to choose from!


Assessment Option #1 Includes:


-Capital Letters

-Lowercase Letters

-Letter Sounds

-CVC words



-Silent E

-Sight Words (Pre-primer - 3rd)

Assessment Option #2 Includes:

-Dolch Sight Words Assessment: (Divided by grade level) Full sheet also included. Pre-primer - Third Grade

-Phonics Assessment: (Letter and letter sounds, Initial sounds, final sounds, medial sounds, phoneme blends, segmentation, rhymes, short vowel sounds, digraphs, blends, long vowels- silent e)

-Math Assessment: (Writing numbers to 120, counting prompts, number identification, before and after, number sequence, counting objects, comparing numbers, 5 frames and 10 frames – subitizing, addition, subtraction, word problems, and shapes.

-Anecdotal notes page

-Teacher Data Recording Sheet

-Binder Cover

There are printable cards to flash for your students, and a way to digitally display them. I would print cards and place them on a ring to easily flip through them and keep up with them. Most of the printable version of this resource is ink friendly, there are a few color images.

As far as the digital version goes, once you make a copy of the assessment, you can remove certain slides that do not apply to your students (For example: I would not use first- third grade sight words for my kindergarten students, so I would make a copy of the resource, without those slides included) etc.
