As a middle school art teacher, I allow well-behaved students to listen to music on their device (with headphones and appropriate volume of course). It was a privilege that was to be earned. But I would get busy and forget who had permission and who had not.
I needed a strategy to go along with the incentive- Beat Breaks were born!

ALWAYS KNOW WHO HAS PERMISSION TO LISTEN TO THEIR HEADPHONES/MUSIC DEVICES and who does not WITH A SIMPLE LOOK AROUND THE ROOM!
PRINT FRONT AND BACK ON CARDSTOCK.
SIGN AND DATE EACH TIME A STUDENT IS ALLOWED TO LISTEN TO THEIR MUSIC.
MY STUDENTS LIKE TO WEAR THEIRS AS NECKLACES – HOLE-PUNCH THE TOP AND ADD YARN.
SIGN AND DATE THE PASS ON THE LINES PROVIDED EACH TIME YOU GIVE PERMISSION TO THE STUDENT
