Use this packet at the beginning of the school year and again at the end! At the beginning of the year, print and staple pages 1-5 for each student, read the paragraph at the top of page two as a class (if needed), and then have each student fill out the questionnaire. Reading each of the student questionnaires will not only help you get to know each one better, it will also help you differentiate instruction based on your students' interests and needs. After students have completed the first questionnaire and you have had a chance to read them, store pages 1-5 for each student in a class envelope until the end of the year. At the end of the year, print and staple pages 6-9 for each student and have each one fill out the questionnaire again. As students complete the end of year questionnaire, give back his or her beginning of year questionnaire to compare and contrast the two, looking for similarities and differences over time. Reuse page 5 at the end of the year, instructing students to retrace his or her hand on top of the beginning of year tracing but using a different color pencil, pen, or crayon to better see the change. New students who join your class during the year can fill out the first questionnaire whenever they arrive. If a student moves out of your classroom, send his or her first questionnaire in a sealed envelope with a blank end of year questionnaire they can fill out later.
